Abstract: Glaucoma is one of the major causes of blindness worldwide. There are many theories regarding the causation of glaucoma, but the current understanding is that of a multi-factorial disorder. However, despite advances in diagnosis and management, the treatment of glaucoma is limited to control of intraocular pressure. In many cases, the patient continues to worsen in his functional and structural parameters, despite apparently adequate control of intraocular pressure. This has led to the focus on alternative and complementary modes of therapy. In Unani medicine glaucoma (maa-e-aksar) is regarded as a disease caused by the derangement of the humors. Therefore, it is a condition not just confined to the eye, but a systemic disorder. Ilaj-bit-tadbeer (Or regimental therapy) treats diseases by various means. Leech therapy has a significant role in treating acute, chronic, and advanced glaucoma. This article reviews our current understanding regarding the role of leech therapy in glaucoma management.
