Abstract: Folliculitis of the scalp is a common inflammatory condition characterized by pustules, itching, and pain. Conventional treatments often provide incomplete relief, prompting exploration of alternative therapies. A 40-year-old male with an 8-year history of scalp folliculitis presented with recurrent pustules, itching, and pain. Previous treatments with antibiotics and homeopathy offered partial but temporary relief. The patient sought Ayurvedic intervention after experiencing persistent symptoms. Based on clinical features, the condition was diagnosed as Pittaj kaphaj Pidika (Pustules), involving an imbalance in Pitta and Kapha doshas, along with Krimi (parasite infestation). The patient was treated with a combination of Ayurvedic oral medications—Gandhak Rasayana, Panchnimba Churna, Aarogyavardhani Vati, Patolkatukrohinyadi Kashaya, and Dashang Lepa (paste). Additionally, leech therapy (Jalaukaavacharana) was administered weekly for four weeks. After 14 days of oral medication, the number of pustules and associated symptoms significantly decreased. Following four sessions of leech therapy, all pustules subsided, and no new pustules appeared over a 10-week period. The patient remained symptom-free during follow-up, and all medications were discontinued after three months.
